No information that you do not wish to share will be collected by the website.
The only data collected is those that you want to share via the Speech Therapy Access Form (Waiting List). They are necessary in order to demonstrate your desire to have speech therapy with the professional and so that she can contact you.
Your information will be stored on Microsoft servers (in Canadian territory) and will be protected by a password that only the SLP has. She is therefore the only person who has access to your personal information.
The website may contain links to external websites where your personal information may be collected. You will need to refer to their privacy policy for more details.
